The Traumatic Brain Injury (TBI) Medicaid Waiver is part of the Home and Community-Based Services (HCBS) program overseen by the New York State Department of Health. This waiver program allows individuals with TBI or related diagnoses to receive care at home or in a community setting instead of a nursing facility.

Enrolling in our TBI Services In New York service is designed to be straightforward and personalized, ensuring that you receive the care you need promptly and efficiently.

Call the RRDC

Reach out to your local Regional Resource Development Center (RRDC) and request to begin the TBI Waiver application process.

Nurse evaluation

A nurse from the RRDC will conduct a home visit to evaluate eligibility and recommend the level of services needed.

Select Agency

If approved, you can select a Service Coordination provider—such as Individual Home Care—to help create your Initial Service Plan (ISP) and begin care.

Do I need Medicaid to qualify?

Yes. The TBI Waiver is a Medicaid program. Active Medicaid coverage is required and must remain current during your participation.

Are there any restrictions to be in the TBI program?

Participants must be between 18 and 64 years old and have a documented diagnosis of Traumatic Brain Injury or a comparable condition.

Is there a cost to be in the TBI Waiver program?

No. The TBI Waiver Program is fully funded by Medicaid. As long as your Medicaid is active, there is no cost to you.
