Discover the Difference
Our certified Home Health Aides (HHAs) are trained to assist with activities of daily living (ADLs), helping you or your loved one maintain independence and comfort at home.
Under the supervision of a licensed Registered Nurse (RN), each HHA follows a personalized Plan of Care tailored to your specific needs. Unlike CDPAP, HHA services are fully managed by our team—including hiring, training, scheduling, and ongoing oversight—so you can focus on what matters most: your health and peace of mind.
Receive support with daily tasks from a certified HHA—all from the comfort and familiarity of your own home.
Each HHA follows a personalized care plan created and overseen by a licensed Registered Nurse.
Our team takes full responsibility for hiring, training, and scheduling your caregiver so you don’t have to.
HHA Enrollment Process
Getting started with Individual Home Care is simple, supportive, and centered around you. Here's what the process looks like:
Schedule an assessment
Contact the Conflict-Free Evaluation and Enrollment Center (CFEEC) to begin your eligibility assessment for Managed Long-Term Care (MLTC). Then, choose the MLTC plan that works best for you.
RN visit
An RN from your selected MLTC plan will visit your home to assess your care needs and determine how many hours of home care services you’ll receive.
Receiving Services
Once enrolled and approved, your HHA care can begin as early as the 1st day of the following month.
Home Health Aide services can be covered through Medicaid, Medicare, private insurance, or private pay.
Eligibility depends on a successful CFEEC assessment. Individual Home Care can help you schedule this evaluation.
You have options. You can appeal the decision or reapply after 90 days. Our team can guide you through either process.
Yes! A quick call to your Managed Long-Term Care Plan (MLTCP) case manager can initiate the switch. We’re here to assist every step of the way.

Eligibility is determined through assessments conducted by programs like the Managed Long-Term Care (MLTC) plan or the Community Health Assessment.
