The Nursing Home Transition and Diversion (NHTD) Waiver is a Home and Community-Based Services (HCBS) program offered through the New York State Department of Health. This Medicaid-funded initiative supports individuals with physical disabilities and older adults who either wish to transition out of nursing facilities—or avoid institutionalization altogether—by receiving services at home or within their community.

NHTD services are ideal when existing Medicaid programs, informal caregivers, or local resources are not enough to ensure a person’s health, safety, or independence.

At its core, the NHTD program embraces the belief that individuals with disabilities and seniors have the same rights as anyone else—to make choices, take risks, learn through experience, and live life on their own terms.

The philosophy of the program is that individuals with disabilities and/or seniors have the same rights and therefore should be in control of their lives, encounter and manage risks and learn from their experiences.

Each participant in the waiver will have a Service Coordinator who will assist them in gaining access to needed services, as well as other local, state and federally funded educational, vocational, social, medical and any other services.

NHTD Enrollment Process

We make the process clear and manageable, guiding you every step of the way.

Call the RRDC

Reach out to your local Regional Resource Development Center (RRDC) and ask to begin the NHTD Waiver application.

Nurse evaluation

A nurse from the RRDC will schedule an in-home visit to determine your eligibility for the waiver.

Select Agency

Once approved, you’ll select a Service Coordination provider—like Individual Home Care—to help create your Initial Service Plan (ISP) and begin receiving support.

Do I need Medicaid to be in the NHTD program?

Yes, the NHTD Waiver is a Medicaid program. You must have active Medicaid coverage and maintain it throughout your participation.

Is there an age requirement for the NHTD program?

Yes. Applicants must be:

  • Between 18 and 64 with a physical disability, or
  • 65 and older at the time of application.

Will it cost anything to participate?

No. If you are enrolled in Medicaid, all services offered through the NHTD Waiver are fully covered.
