Home Health Aides (HHA) In New York

Personalized Care. Right at Home.

Discover the Difference

Our certified Home Health Aides (HHAs) are trained to assist with activities of daily living (ADLs), helping you or your loved one maintain independence and comfort at home.

Under the supervision of a licensed Registered Nurse (RN), each HHA follows a personalized Plan of Care tailored to your specific needs. Unlike CDPAP, HHA services are fully managed by our team—including hiring, training, scheduling, and ongoing oversight—so you can focus on what matters most: your health and peace of mind.

HHA Enrollment Process

Getting started with Individual Home Care is simple, supportive, and centered around you. Here's what the process looks like:

01

Schedule an assessment

Contact the Conflict-Free Evaluation and Enrollment Center (CFEEC) to begin your eligibility assessment for Managed Long-Term Care (MLTC). Then, choose the MLTC plan that works best for you.

02

RN visit

An RN from your selected MLTC plan will visit your home to assess your care needs and determine how many hours of home care services you’ll receive.

03

Receiving Services

Once enrolled and approved, your HHA care can begin as early as the 1st day of the following month.

Frequently asked questions about HHA

How do I pay for a Home Health Aide?

Home Health Aide services can be covered through Medicaid, Medicare, private insurance, or private pay.

If I have Medicaid, do I qualify for an HHA?

Eligibility depends on a successful CFEEC assessment. Individual Home Care can help you schedule this evaluation.

What if I don’t pass my CFEEC assessment?

You have options. You can appeal the decision or reapply after 90 days. Our team can guide you through either process.

Can I switch from CDPAP to HHA services?

Yes! A quick call to your Managed Long-Term Care Plan (MLTCP) case manager can initiate the switch. We’re here to assist every step of the way.

As a homecare provider myself, I cannot say how impressed and happy I am with Individual Home Care. In the world of home care it is important to be hands on, caring, compassionate and readily available if and when there are issues. Charles and his staff are all of those things. I have gotten to see Charles on many occasions speak with our mutual clients and clients of his own. It is always done so with care, real time advice and direction, and a top priority to Charles and staff. They are able to smoothly navigate Medicaid and any bumps in the road that come with the process. I strongly recommend every and all of our clients to Charles with extreme confidence on a daily basis. Thank you Individual Home Care!!!!!

I recently switched my agency to Individual Home Care. I was with my last agency for 13 year for my 35 year old son. What I thought would be a stressful transfer was not! The staff at Individual are phenomenal and I can’t say enough good things about the HHA’s. They are so kind, compassionate, and caring. Thank you Individual!!!
